Job Role

Catalyst Choices are seeking dedicated and compassionate Care Leader to join our team to support our 10-bed Intermediate Care Unit, alongside our OP residents in our care home.

This service will support patients who are coming out of hospital and require a period of rehabilitation, and they will stay with us as guests at Woodleigh care home.

You’ll play a vital role in supporting individuals with their day-to-day needs, promoting independence, dignity, and wellbeing.

Your role will include building meaningful relationships, planning and supporting engaging activities, and ensuring each person receives personalised care that reflects their choices and preferences.

Responsibilities:

• Deliver high-quality, person-centred care and support.

• Supervise and coordinate the care provided in our supported tenancies.

• Develop and implement individualized care plans for each resident.

• Communicate effectively with staff, residents, and their families.

• Monitor the health and well-being of residents, including medication management.

• Oversee daily activities and ensure a safe and comfortable environment for residents.

• Provide guidance and support to care staff.

• Collaborate with healthcare professionals to ensure the best possible care for residents.

• Maintain accurate records and documentation.

What do you need to join us?

• Previous experience in care or a related field is desirable but not essential.

• A caring, empathetic nature and a genuine passion for helping others.

• Good communication skills and a team-focused attitude.

• Willingness to learn and grow within the role.

• Basic IT skills for documentation purposes.

• Ability to work under pressure and handle challenging situations with professionalism.
